Our 2021 conference has something for everything for one really competitive price.* This year everyone who pays a full conference registration can attend the in-person event at no additional cost (other than travel/hotel). Pandemic Pricing is also available once again for the virtual conference. *ELOs require an additional purchase.
Extended Learning Opportunities
Tuesdays and Thursdays from September 14th – October 14th via Zoom
Extended Learning Opportunities (ELO) are deep dives into specific content areas – kind of like mini-conferences held right before our big one. We are offering 7 ELOs this year that will take place before the annual conference with really competitive pricing. Each ELO is $49 for trainees and $99 for professionals. If you purchase three ELOs you get access to all seven!
CFHA Leadership Day
October 15th in-person at the Monona Terrace in Madison, Wisconsin
The goal of this 1-day leadership event is to gather as a community of integrators to develop a guidepost document that details the changes we believe necessary to advance integrated care nationally and in the state of Wisconsin. If you are a leader, interested in becoming a leader, or are just interested in hanging with the best minds in integrated care, this is the event for you. (Hint: If you belong to CFHA, you are a leader…) Anyone at any level is welcome to network and reconnect with the community in this highly interactive in-person experience.* Stay tuned for more information.

SharedCare Canada Conference Day

This year, CFHA is proud to co-host our conference with our Canadian sister organization, SharedCare. On October 19th, our Canadian colleagues will be presenting concurrent sessions and a plenary related to this year’s conference theme, “Justice, Health Equity and Reducing the Digital Divide”. CFHA attendees are welcome to attend.

Similar to last year’s virtual conference, this year’s conference will be filled with sessions, podcasts, webcasts, discussion groups and a series of plenary sessions. This event will include a mix of CFHA and SharedCare sessions related to this year’s theme, “Justice, Health Equity, and Reducing the Digital Divide.”
