Summary
Now, more than ever, we are facing the realities resultant from socioeconomic and racial disparities. Over the past 6 years, UW Health has worked in a systemic fasion to close the gaps and bring equitable healthcare to all. This symposium will discuss the actionable steps that occured on the institutional departmental (psychiatry and primary care) and provider levels to address these barriers and increase mental health access to our medically-homed patients. This workshop will also discuss how telehealth and virtual platforms were leveraged during the pandemic to ensure sustainability of our integration efforts.
Objectives
- Identify how telehealth affected the warm handoff at UW Health
- Discuss the effectiveness of proposed solutions to decreased handoffs
- Describe creative solutions to doing handoffs in a virtual space
