Summary
Participants will get information on our our same day clinic, with 3 medication providers and one behavioral health consultant, helps see patients on a walk in basis to ensure comprehensive care. This model is intended to decrease Emergency Room usage and use appropriate levels of care. We will showcase 2 case studies to showcase how this help decrease emergency room usage, help patients get necessary medications and provide resources to decrease homelessness and increase access to care.
