Summary
Forte Wellbeing, a Phoenix-based multidisciplinary and holistic medical home, has designed lifestyle group programs and standard workflows to be grounded in community and individuals’ capacity for behavioral change. According to learning theories, participating in a new, complex behavior, such as moving from a sedentary to active lifestyle, typically requires learning small components of a complex behavior due to physical, mental, emotional, social and environmental barriers that complicate lifestyle change. Forte’s care model is designed to break down barriers to health behavior change by aligning patients with the appropriate services and providers while considering patients’ readiness to change, the context of their life and the complexity of their health needs. Forte is based upon world class improvement methodologies (Lean) with data-based decision making and utilization of analytics to create an operationally sound, innovative and scalable care model which puts the patient at the center of a reimagined medical home.
Objectives
- Describe a collaborative care model based upon Lean methodology for scalability utilizing both insurance based, and cash pay options.
- Identify components of a multidisciplinary care model that mitigate physician and clinician burnout while improving clinical care outcomes.
- Meeting patient needs within a holistic medical home utilizing occupational and physical therapy, dietitians, medical providers, yoga and coaching.
